本論文企圖綜合當代精神醫學診斷、心理學文獻，民族誌資料以及國內臨床個案等討論，詮釋思覺失調症的病理徵候和醫療處遇，以及患者的受苦經驗和臨床照顧。本論文透過具備臨床訓練的人類學觀點，以衛生福利部某精神科專科醫院作為一項民族誌研究，試圖提出思覺失調症病理徵候的詮釋觀點，強調思覺失調症患者的妄想思緒和混亂行為具備內在邏輯性；呈現思覺失調症患者的生病經驗，同時彰顯患者在精神醫學文化脈絡下的主體論述；最後提出精神病院對於患者的臨床照顧以及機構安置的意義。

本論文透過Karl Jaspers心理病理學的意義詮釋、醫療／詮釋人類學的文化體系概念，以及精神病院的臨床民族誌調查三項書寫主題，提出臨床作為文化體系、精神疾患患者的功能光譜、臨床脆弱性，以及臨床多樣性等原創觀點。本論文透過具備臨床訓練的人類學觀點，以精神病院作為一項民族誌研究，期許此一臨床民族誌之研究貢獻與預期影響，得以彙整生物醫學觀點、病院管理機制批判、患者主體經驗等論述，為精神醫學處遇與人文臨床照顧增進彼此知識體系的觀點，並且得以協力開展臨床社會科學。

The thesis attempts to synthesize contemporary psychiatric diagnosis, psychological literatures, ethnographic accounts, and clinical cases, in order to interpret the pathological symptoms of schizophrenia, medical treatments, patients’ illness experiences, and clinical care in a psychiatric hospital, Taiwan. The project emphasizes the delusional disturbances of schizophrenic patients in referring to their intrinsic logic. It further deals with the issues of symptoms and illness experiences, thereby reconsidering the treatment and intervention for clinical reflections in modern psychiatry.

Drawing Karl Jaspers’s psychopathy on his interpretation of meaning as well as medical and interpretive anthropology on the conceptualization of “cultural system,” this project as a clinical ethnography elucidates insightful theses including clinic as cultural system, the spectrum of patient’s functional scale, clinical vulnerability, and clinical diversity. As a result, this thesis conducted by an anthropological professional with clinical trainings in a psychiatric institution as an ethnographic fieldwork, intends to integrate biomedical science, total institution of mental health, narrative subjects, and clinical care, into a study of “clinical social science.”
