With the improvement of health awareness, various dietary principles have been proposed. However, being too obsessed with eating healthy and restricting food intake may become pathological. This study aims to develop a suitable tool to measure the cognitive behavioral pattern of orthorexia nervosa (ON), and its impaired performance in physical, psychological, and social aspects in Taiwan. We collected and translated past orthorexia scale items, recompiled them into Taiwan Orthorexia Nervosa Scale (TWOS), and verified the reliability and validity. The study was conducted in two phases. Study 1 recruited 308 samples aged 20-59 to establish the factor structure through exploratory factor analysis. Study 2 recruited 222 samples aged 20-59 to confirm the factor structure model, internal consistency, convergent validity, and stability after 2 weeks. The result supports a 21-item scale with 3-factor model labeling (1) Belief and Behavior of Orthorexia, (2) Health and Life Problems of Orthorexia, and (3) Emotional Distress of Orthorexia. The scale has good internal consistency, stability reliability, and adequate convergent validity. Furthermore, we measured the association between TWOS and other measures and found that ON tendency was significantly correlated with eating disorders and perfectionism. Consequently, the current study suggests that TWOS is a suitable tool for measuring ON tendency in Taiwan.
