由源自中國武漢的嚴重急性呼吸系統綜合症冠狀病毒2臨床症狀(SARS-CoV-2) 引起的2019年冠狀病毒病(COVID-19)大流行是世界衛生組織(WHO)宣布的持續性突發公共衛生事件。長期照護機構為人口密集高風險感染之場所，易導致群聚傳染，長期照護機構對內應維持住民良好衛生習慣與生活方式對外如機構訪客管理、疑似病例之轉介、配合社區的防疫作為等，並增加抗生素使用、隔離照護、提升管路照護、住民外出限制、疫苗施打等措施，對住民感染率下降是否真有顯著差異，引發研究動機。本研究採橫斷式回溯性研究，探討長照機構因應COVID-19感染管制措施介入後，對長照機構住民醫療使用門診、急診及住院醫療資源使用現況，選取108年01月01日至109年12月31日期間，花蓮市某長期照護住宿式服務機構150位住民病歷紀錄做為研究資料來源。經本研究之研究假設驗證結果為：
1.去除感染源（抗生素服用）對門診、急診及住院醫療使用有顯著差異。
2.切斷感染途徑之隔離照護對門診醫療使用有顯著差異。
3.切斷感染途徑之管路留置對醫療使用無顯著差異。
4.切斷感染途徑之外出限制對門診醫療使用有顯著差異。
5.增加免疫力（疫苗施打）對門診醫療使用有顯著差異。

The Coronavirus Disease 2019 (COVID-19) pandemic caused by severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) originated from Wuhan, China, is an ongoing public health emergency declared by the World Health Organization (WHO). Due to the high risk for communicable diseases transmission within Long-Term Care Facilities, reinforced practices for infection prevention and control are warranted during the COVID-19 epidemic. Besides abiding by government regulations, multiple internal and external approaches for infection prevention and control within Long-Term Care Facilities were implemented. However, the effectiveness of the mentioned measures was unclear. This was a cross-sectional study aiming to analyze the effectiveness of the infection prevention and control measures implemented since December 2019 in a hospital-based nursing home in Hualien, Taiwan. Data were collected from the medical records of residents between 1st January 2019, and 31st December 2020. The main outcomes were numbers of outpatient visits, emergency department visits, and admission to the hospital due to respiratory tract infection. Results are as follows:
1.There are significant differences in the use of antibiotics for outpatient, emergency and inpatient medical use.
2.There are significant differences in the use of isolation care for outpatient medical use.
3.There is no significant difference in tubing placement for medical use.
4.There are significant differences in out-patient medical use due to restrictions on going out.
5.Vaccine administration has significant differences in outpatient medical use.
